United Way of Sumner County is converting to Electronic Funds Transfer (EFT), instead of mailing paper checks for Allocations/Designation payments to our partner agencies.This will result in a more efficient and cost-effective method of forwarding funds.
To enroll for EFT you will need to complete our Electronic Funds Transfer Authorization Agreement. Please return by fax, mail or email.
COMPLIANCE DOCUMENTS
If you are a United Way partner agency, you will be asked to complete the two documents listed below on an annual basis. You must also review the Partner Agency Policy Manual to comply with the requirements of the Agency Agreement. Failure to return completed forms by the due date will result in your funds (allocated and designated) being held until the paperwork is completed and returned to United Way of Sumner County.
PLEASE NOTE: by Friday, July 23, all 2009-2010 partner agencies must complete and return their Outcomes Final Report to United Way of Sumner County. The document is listed below.
If you are an agency receiving only designated funds from United Way of Sumner County, you are required to complete the Patriot Act compliance form ONLY. Failure to do so will result in your funds being held by United Way of Sumner County.
United Way of Sumner County must have these documents on file for every partner and non-partner agency receiving funds, as part of its membership and compliance requirements with the United Way of America.
